YOUR CHOICE gives individuals or their family/guardian the ability to hire a support worker of their choosing. This can be a friend, a family member, or even a neighbor!
You control your care by working with your support worker to coordinate work schedules and develop a personalized support plan based on your unique wants and needs.
You decide who and what is right for you.
Anyone who is enrolled in the Ohio Developmental Disabilities system and has one of the following is eligible to participate:
There is no cost to you to participate in this program.
Identify a person who is a good fit to become your support worker.
Fill out the YOUR CHOICE application.
Once your application is received, Envision staff will reach out to collect more information.
Envision will handle the employee onboarding process for your new DSP. This includes eligibility verification, background checks, training, and payroll matters (competitive wages starting at $18 per hour).
Once the information is processed and training is completed, your new DSP will be eligible to begin providing support services. Be sure to keep your SSA informed as well.
